NOTICE TO PUBLIC:

            PCHS announces its recognition of becoming a Patient-Centered Medical Home designation.

     The Patient-Centered Medical Home (PCMH) is a care delivery model whereby patient treatment is                    coordinated through their primary care physician to ensure they receive the necessary care                                                 when and where they need it and in a manner they can understand.

The objective is to have a centralized setting that facilitates partnerships between individual patients and their personal physicians, and when appropriate, the patient’s family. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.

Welcome to Preventative Care Health Services. Our mission is to provide quality medical, dental, and behavioral health services that are accessible while caring for our communities. We strive to decrease barriers to heath care, improve our patient's health, and enable our staff in an innovative and collaborative journey to a healthier future. We are designated as a Federally Qualified Health Center (FQHC) and are a Federal Tort Claims Act (FTCA) deemed facility.


This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
